Description: We report an outbreak of vancomycin susceptible vanA+ enterococci able to escape phenotypic detection by current guidelines, found by two initial cases of in vivo switching into vancomycin resistant Enterococcus faecium (VREfm) in vancomycin-treated patients and a subsequent screening initiative. Six isolates were sequenced by Illumina MiSeq, of which two vancomycin susceptible vanA+ isolates from infected cases and two vancomycin resistant isolates stemming from the same two patients after vancomycin treatment. We also provide two vancomycin susceptible isolates from a subsequent screening initiative.
